Before you come in
We always want patients to feel informed and confident they are getting the best treatment possible. Everything you would normally have to hunt for is here.
Your first visit begins with a comprehensive oral evaluation, and we build a treatment plan from it. Bring your dental insurance card, a list of any medications you take, and arrive early if your paperwork is not already done.
If you need antibiotics before dental work, take them as prescribed. If you are not sure whether you need them, contact your doctor before the visit.
Two forms are worth completing before you arrive: the Patient Health History Form and the Notice of Privacy Practices. Print them, fill them in and bring them with your insurance card.
If you cannot print them, plan to arrive 10 to 15 minutes early and complete them in the office.
Dental insurance is not medical insurance, so it is worth reading the benefit booklet your provider sent you. We are out of network, and we will give you every form and document you need to be reimbursed promptly.
We accept cash, debit cards, Visa, MasterCard and Discover. Please give at least 48 hours notice to reschedule; under 24 hours may carry a cancellation charge.
Keeping the mouth free of harmful bacteria, infection and inflammation helps stop germs spreading further. Gum disease appears to have links to diabetes, osteoporosis, respiratory disease and some cancers.
At every visit we watch for signs of oral cancer, sensitivity that makes chewing hard, and evidence of grinding, snoring or sleep apnoea.
Written instructions for the procedures that need them. Ask at the desk for a printed copy, or call if something does not look the way you expected.
During office hours, immediate attention is given to your situation and you will be seen as soon as possible. After hours, call the office and follow the instructions provided. If you are experiencing a serious or life‑threatening emergency, call 911 or go to the nearest emergency room.

We are considered out of network. That does not mean you cannot use your benefits: we provide every form and document your insurer requires so you are reimbursed promptly. Dental insurance works differently from medical insurance, so it is worth reading the benefit booklet your provider gave you before your visit.

Invisalign®. A custom series of smooth, virtually invisible aligners you wear over your teeth and change roughly every two weeks, with no brackets and no wires. Dr. DeWaele is an Invisalign Premier Provider, and there are options for children, teens and adults.
Early, and then every six months. Primary teeth aid speech development, let food be chewed properly and hold space for the permanent teeth, so they are worth looking after. Visits include cleanings, sealants, fluoride and digital imagery to check on what is coming through.
Yes. The building at 208 South Avenue is designed with ample parking, with additional parking around the back. There are Saturday morning hours and we open at 7:00am on weekdays, so an appointment does not have to cost you a working day.
